Step Three: Fill out the Affidavit Form

Completing this affidavit form isn't as hard
as it sounds. Mostly name, address, that kind of stuff. Also if you have an attorney, make sure your
attorney's information is correct because your required briefing certificate depends on it and the
courts can be hard on receiving incorrect information. Of course your certificate may be faxed
and/or mailed to your attorney, and we don't want your information going to the wrong place.
