PRIVACY NOTICE TO OUR CUSTOMERS At Family Life Resources, Inc.
protecting our clients’ privacy is paramount; we recognize that an
individual’s financial matters are extremely private and sensitive in
nature. Our commitment is to maintain strict standards to safeguard your
personal and financial information at all times, while diligently seeking to
continue to bring you valuable products and services.
This information is being provided to you as required by the Gramm-Leach-Bliley
Act 1999 (Public law 106-102) and the Federal Trade Commission ruling dated
July 1, 2001.
INFORMATION WE COLLECT and SHARE
The personal financial information we collect directly from you includes
information required to communicate with you and to assist us effectively to
address your financial needs, as well as, to operate prudently in complying
with the legal and regulatory requirements imposed by federal and state laws
and regulations.
Such personal financial information we may collect and share is made
available to us from the following sources:
- Information received on applications and other forms, such as your name,
mailing address, telephone number,
e-mail address, fax number, social
security number, birth date, etc.
- Information received directly from you through face to face conversations,
phone calls, e-mails and correspondences.
- Information about your transactions with us. Such as accounts, balances
owed, creditors names, account status, rates, payments made and legal
ownership.
- Information we may obtain about you from a consumer reporting agency, such
as your credit score and credit history.
- Information we receive from your creditors.
In all cases, we collect and share only personal financial information that
is either required by law or necessary to provide personalized financial
services to you.
OUR USE OF YOUR PERSONAL FINANCIAL INFORMATION
In the course of conducting consumer credit counseling services on your
behalf, there are occasions where we may engage affiliated and/or
nonaffiliated companies to (i) service or process a financial product or
service requested by you or, (ii) to maintain or service your account(s).
These companies may be engaged in a related business such as a creditor,
mortgage lending, bankruptcy, credit audit and verification, tax services,
insurance products, auto products, etc. In such cases, the personal financial information we may share about you
with these companies is necessary and permissible by law to administer,
process, service and enforce a transaction that you requested or authorized. We may also share some or all your personal financial information with your
legal representative, trustee or other fiduciary, or where we need to
protect the confidentiality and security of our records, or to prevent
potential or actual fraud, unauthorized transactions, claims or other
liability. Also, as permissible by law, we may provide your personal financial
information on request to our attorneys, accountants, and auditors. To
others also, such as enforcement agencies, consumer protection agencies,
consumer reporting agencies or to comply with federal, state or local laws.
Outside of the exceptions mentioned above and as permitted by law, we will
not share your personal financial information with anyone, unless you
specifically authorize us to do so.
CONFIDENTIALITY AND SECURITY
When we share your personal financial information with affiliated and/or
nonaffiliated companies, they are not allowed to use your personal financial
information for their own purposes and are obligated to maintain strict
confidentiality. We limit their use of your personal financial information
to the performance of the specific product or service we have requested.
We restrict access to your personal financial information to employees who
need to know such information in providing products or services to you. We
maintain physical, electronic, and procedural safeguards that comply with
applicable federal regulations to guard your personal financial information.
Of course, even with these safeguards, we cannot guarantee that your
personal financial information will always be safe from unauthorized
sharing, and we assume no responsibility or liability in that event, except
as and to the extent that may be imposed by law.
According to the federal trade commission ruling, we are not required to
provide an opt out right for information to be shared with a consumer’s
creditors.
OTHER IMPORTANT INFORMATION
If you decide to close your account (s) or become an inactive customer, we
will adhere to the privacy policies and practices described in this notice.

CUSTOMER DISCLOSURE STATEMENT

Customer Disclosure Statement
- In accordance with the Bankruptcy Abuse Prevention and Consumer
Protection Act of 2005 (Public law 109-8: section 111 (2) (D), we
present to you the following information:
Family Life Resources is a 501 (c) 3 non-profit Christian Credit
Counseling Agency.
- Our main office is located at 5802 E. Fowler Ave, Tampa, FL 33617.
Phone: 813-989-1900. Fax: 813-989-0359. Website: www.flrministry.org.
Business hours are from 9:00 AM to 5:00PM. Evening counseling by
appointments only.
- We provide initial and general counseling at no charge to our
customers. The outcome from this counseling will not include the
certificate.
- Family Life Resources offers Pre-filing Bankruptcy Counseling in
telephone/internet, telephone and in-person format. This fee is for the
combination of the counseling and the certificate. The fee for the
telephone/internet counseling is $25 per person. The fee for the
telephone or in-person counseling is $50 single and $75 married filing
joint. In accordance with the Interim Final Rule, we also offer this
counseling for free or at a reduced rate based on a client who lacks
ability to pay. If your household current income is at or below 150% of
the poverty income guideline or your attorney provided pro bono service
for your bankruptcy case, then please contact us for this free or
reduced rate service. Proper verification is required.
- Our counselors have completed a
national certification course from AFPCE. This certification is
nationally recognized and has prepared our counselors to provide
appropriate credit counseling. In addition, our counselors complete a
six month in-house training program.
- We receive donation money from some creditors. This is known as
“Fair Share”. This donation is not subtracted from client payments to
the creditors but comes directly from the creditors. We also receive
money from services rendered, donations from the community and grant
money awarded from public and private sectors.
- We are governed by a Board of Director. This governing body must
have the majority of which are not related and or are not employed by
Family Life Resources. In addition, no board member shall receive any
commissions, incentives, bonuses or benefits of any kind based on the
outcome of a counseling session.
- Each customer is provided with a Notice of Privacy brochure in
compliance with the Graham-Leach-Bliley Act of 1999 (Public law 106-102)
and the Federal Trade commission ruling dated July1, 2001.
- Family Life Resources does not report any information to any credit
bureau. We are not responsible for any information reported to any
credit bureau by your creditors. If a client participates in a DMP,
creditors may report that client’s debt was compromised or that client
is participating in a DMP. Each creditor is free to interpret these
statements as they see fit. Some will look favorably upon participation
in a DMP, while others may look upon it negatively.

Student Disclosure Statement
In accordance with the Bankruptcy Abuse Prevention and Consumer

format and on-line internet format. The fee for the Internet program is
$25.00 per person. The fee for the Classroom program is $50.00 per
Household. In accordance with the Interim Final Rule, we also offer this
counseling for free or at a reduced rate based on a client who lacks
ability to pay. If your household current income is at or below 150% of
the poverty income guideline or your attorney provided pro bono service
for your bankruptcy case, then please contact us for this free or
reduced rate service. Proper verification is required.
- Family Life Resources does not pay or receive fees or other
consideration for the referral of students to Family Life Resources.
- Upon successful completion of the education course, Family Life
Resources will provide a certificate of completion to the student.
- Our instructors have completed or in the process of completing a
national certification course from AFPCE. This certification is
nationally recognized and has prepared our counselors/instructors to
provide appropriate credit counseling and education. In addition, our
counselors/instructors complete a six month in-house training program.
- We are governed by a Board of Director. This governing body must
